Forms for registration

During the registration process, be prepared to upload the following documents:

1) 3 Proofs of residency within GNO boundaries (driver's license, homeowner or tenant
records, utility bills) all dated between February 1, 2025 and February 1, 2026.

OR

A form verifying attendance at a school within GNO boundaries. The school enrollment
form can be found here. The school enrollment form if your child attends school within our boundary map is the easier paperwork for the parent.

2) Birth certificate of participant

3) Medical release form. The medical release form can be downloaded here.
